To file a complaint against a facility using our online portal, you will need at least one of the following search criteria:

• Facility/Agency name
• Facility/Agency Address(Street, City, or County)

If you DO NOT know the name or address (either Street, City, or County) to be able to utilize the online portal, then please email complete details of your complaint along with contact information* to RCCreports@dhs.ga.gov

*Please note, your contact information is kept confidential and is required if you want to be contacted or get a summary of the results of the investigation.
